- I will deliver sessions in a manner consistent with the mission, values, and standards of DWF Nigeria.
- I will represent DWF with integrity, professionalism, and respect for all participants.
- I will prepare adequately using DWF-approved materials, templates, and guidelines.
- For Diabetes GEMS sessions, I will cover a minimum of 2 GEMS domains appropriate to my audience.
- I will meet minimum session durations: 45 minutes for GEMS; 20 minutes for awareness sessions.
- I will submit a Session Plan Form and Budget Request at least 5 days before every session.
- No DWF session may be delivered without prior registration and approval.
- I will maintain an accurate Attendance Register at every session.
- I will capture a minimum of 3 photos at each session.
- I will submit a Post-Session Report within 72 hours of every session.
- I will use DWF funds only for approved purposes.
- I will submit accurate, genuine receipts for all expenditure.
- Fraudulent claims will result in immediate suspension and removal from the programme.
- I will not provide clinical diagnoses or prescribe medication to any participant.
- I will refer clinical concerns to an appropriate healthcare professional.
- I will handle all participant information with discretion and respect.
- I will follow DWF social media guidelines when sharing session content online.
- Providing full DEFP onboarding training and a Facilitator Resource Pack
- Supplying DWF-branded materials and presentation slides
- Reviewing Budget Requests within 3 working days
- Releasing approved funds before each approved session date
- Issuing an official DWF Facilitator ID Card upon onboarding completion
- Recognising facilitator progress through the DEFP Tier System
- Providing a reference from the DWF Executive Director for facilitators in good standing
This agreement takes effect from the date of signing. Either party may withdraw by providing written notice. DWF reserves the right to suspend immediately in cases of serious breach.
